I don't know of a map or list...but we've got two of them at work. One is outside by the bike racks, other is inside the parking garage by the bike racks. Both have tool stands and bike hangers...I posted pictures of them in my photos a while back. Inside the building lobby(600 W. Chicago), There's a vending machine full of various bike accessories including tubes, lights, locks, tools, etc.

I put all the ones I could find mention of in the bike shops layer of massup a few years back. Find service locations nearest you using http://massup.us/service

At the time there were only 7 (vs 196 shops). Here's an extract.

That 600 W. Chicago vending machine sounds awesome. I love the one on the LFT.

Their popularity has grown and I now regularly see them at unrecorded (by me) locations. Often near bike friendly bars. Gonna follow this thread, and someday I'll update. Help welcome.
