The CDOT bike program did this in 2008 and will do it again this year/early next year, as part of the Streets for Cycling Plan 2020.

It seems like there are tons of bad sections on Damen, starting from 6 corners and continuing well into the UIC area past Lake and Madison, etc.
Thank you to everyone for feedback regarding bikeways striping in need of repair. I will be working with "carfreecommuter" to keep track and review all of the suggestions made as CDOT moves forward with bicycle improvements.
Beginning in 2012, CDOT will be evaluating the entire existing bikeways network and upgrading wherever needed. This will be a major focus for the next 3 years along with creating new facilities, such as protected bike lanes.
I welcome input from from your experiences on the City's bikeways network. I will be providing updates on chicagobikes.org as we move forward.

South Shore Drive. Nearly all of the paint is gone. The surface is in poor condition too.
Elston Ave needs lines repainted and surface maintenace west of Ashland. I never know if I'm in a bike lane or just biking into broken sewer/mainhole covers.
Also Elston just north of Milwaukee/west side of street after the train over pass - broken surface/manhole.
Street is currently being repaved, but I don't know if that surface will be fixed. Also workers are not wearing dust-face masks and their work is kicking up alot of dust (concrete, dirt. asphalt). OSHA should be called in.

Also... east of State on 31st is a death trap (signs state it's a bike route toward the lakefront trail but there's not a single sign for a bike lane on the pavement or lines - maybe there aren't supposed to be? but the broken glass, two-lane, one-way size without defined lanes, and foot deep holes make it a treacherous bit of roadway regardless).
King Dr. between 47th & Cermak has places where the paint is fairly worn. There are also some bad craters.
The same is true on sections of 83rd St. between Vincennes and Stony Island. Pavement condition is more of an issue than paint condition in most places. This used to be a great bike route, but the pavement has gotten bad over the last few years.
I'll keep my eyes open for other locations.
