You've seen the question - here's what I pack:

One (1) multi-tool Swiss Army Pocket Knife
One (1) multi-tool Park Allen wrench set
Three (3) plastic Park Tool tire levers
Two (2) 700x25 Spare Tire Tubes
One (1) Ultraflate Plus CO2 inflator
Three (3) 16gram CO2 cartridges
Three (3) Park Tool Tire Boots
One (1) Park Tool portable spoke wrench
One (1) Typical Tire Patch Kit (Patches, Vulcanizing Fluid & Sandpaper)
One (1) "Drug Kit" containing Band-Aids, Antibiotic Ointments, 4 Tabs of Advil & 6 Antiseptic towelettes)

What am I missing? What should I add?

Pretty much ditto except i prefer a framefit pump to the CO2 -a long pump can double on dog defense. Add in a good compass if you're going 'way out in unfamiliar territory. Someone else recomends a minimum $5 bill. This all (except the pump of course) goes under the seat. Cell 'phone, inhaler, and ID go in the jersey pocket. i ususally don't carry med kit but do reading glasses as i'm pretty much blind inside 5' and don't do well repairing by Braille.

Arm/leg warmers can be stashed in a jersey pocket.

For long hauls and sketchy weather where weight and fashion isn't so much a concern, i like to use a Karrimor saddlebag to stash raingear or extra jersey. Traditional sadlebag and bar bags come in handy.
Sounds kitchen-sink, but it beats hell out of getting stranded in bum****.
i prefer a pump as well. i carry similar to your list, brian. add to that some extra batteries for the lights, a small amount of duct tape, and a mini leatherman tool w/ needlenose pliers.

and minus the fanny pack.
My list is a bit smaller. I carry 4, 5, and 6mm allen wrenches, a 6" adjustable wrench, Gerber clutch, two tubes, patch kit, 35mm film canister of needed meds, spare bulbs, and snacks. I also use a frame pump.
