Witness needed- Collision at Division under 90/94

I'm trying to help a bicyclist out with a pretty horrific bicycle/truck collision that occurred on Division at 90/94.  It is my understanding that there was a bicyclist present who may have seen the collision, so I thought I'd give the Chainlink a shot.
