Yes to the washer.  Triples use the same bolts/washers as square taper doubles.  Circa 2005, there were 3 levels of bolts.  PN FC-RA002 for Mirage, Veloce, and Xenon.  PN FC-CE004 for Centaur.  PN FC-RE104 for Chorus and Record.  The washers were included with the bolts.  These were functionally the same, just had a slightly diffferent appearance.  You can use other brands of bolts too.  The washer protects the face of the crank arm from galling by the bolt head and insures that the arm is pulled onto the taper properly when the correct torque is applied to the bolt.

If you only have foot pound torque wrench, divide the inch pound specs by 12 to get the value.
