What are your methods? If it's small enough to fit in a bag, there's always a chance of leakage. If it's a big box (say a pizza box or a dozen donuts), has anyone had luck affixing it to a rack w/o squashing?

Thoughts?

just put it in your lap
I assume you mean 'take out' or 'carry out'-- food you're purchasing and taking home for yourself vs. a delivery gig?

IMO baskets are the way to go. No need to strap things down, plenty of room for padding and leakage is no big deal. The ride for delicate foods can be a bit jarring-- I have a copy paper box with sheets of padding cut from an old sofa cushion (Fruitcase tm) I use to pack fruit from the grocer.

The double sided rear basket I have on my utility bike creates a nice square platform for carrying a pizza box.

Hmmm.... Pizzzzzzaaaaaghhh.
Yeah, I'm talking more about take out. A full time delivery gig would certainly allow for some good contraptions. I'd probably affix a large insulated pizza holder on the back rack for something like that. I'm talking more the occasional take out/pizza on a more standard bike setup.

I've done regular take out in my Banjo Brothers backpack before, but if there's anything else in there (and there usually is), it risks getting leaks on it.

Of course, I built a cross bike, and a small one for a small guy like me, so I don't know if I have the clearance for a front basket up there.

When you affix the pizza to the back, TC, how do you keep the bungees from a) being tight enough to hold the pizza but b) loose enough not to squash the box into the pizza?

Yeah, at least the places I bought from (Bennison's in Evanston). I was bringing donuts to work for everyone, and I had the hardest time not squishing the box with my net bungee.

yeah, you can do pizzas on a rear rack, just use a loose bungee.. For donuts, use either a basket or a milk crate if it will fit. Most places use flat boxes for donuts now though don't they?
I have done pizza boxes atop a milk crate with two perpendicular bungees. But Like MARK notes, doughnuts are flat packed in a flimsy box. I usually just suffer the consequences there and would be interested in hearing how others do it.

I recently tried to bring home a triple layer tiramisu with an elaborate network of bungees. It was the most delicious disaster ever.
If you like your cheese and sausage stuck to the top of the box, go ahead, strap it down.
